After registering university’s human resources, I had a short meeting with musculoskeletal related three professor’s concerning this mission in this year. I took part in graduate student’s meeting. They discussed mutual researches informatively. Since two professor talked educatively and rapidly as I expected, for now I did not realize how much percent what they discussed. And clinical case presentation started this morning from 7AM, I remined me of my similar experience in my University hospital time. One Prof stimulated me, by her educative talk to graduate student. Her talk to graduate student is informative and educative. I understand her idea for referee and peer review has lots of information. Some students discussed that Core stability and Vibration exercise aren’t evidence based. Maybe I think they may be right. But I’ve been working at University hospital for about 20 years. From the standpoint of clinical situation, we have lots of things we have never known. And for example, in the beginning of LASIK operation, very few doctors only could do it. Nevertheless, nowadays the operation is quite familiar with us. As a physical therapist, I doubt that we have a example like LASIK operation. I would like to create a new thing for the rest of my life, even it will not be a realistic thing.
I have used the 3D motion capture system for over 20 years, but it has lots of problem. One main problem is skin movement artifact concerning the inconsistency of movement between bone and skin, even if we use new complex mathematical methods. But we can see the sliding movement in between subcutaneous tissue and superficial fascia layer during joint motion or passive skin movement by ultrasonography. In my opinion, skin movement is quite natural during the joint movement and it is a crucial of Skin Movement Artifact. Skin movement may have some rules and also have clinical effect. But as you know and I know, these phenomenon contains lot of suspicious things especially for clinical usage for physical therapy. In Japan, I thought our focus tend to direct to the technics but not lack of evidence. That is lack of our meta-cognition bird’s eye view. One of the reason is that we are not familiar with English so we may not know world-wide trend and consequently have a direction to Galapagosizaion. I think we Japanese physical therapist need to know the techniques more evidence based, of course me too. But as a physical therapist, we should know the concrete technique for the patient. I think it’s a Ground rule. So I would like to learn lots of things all of you have done naturally evidence based. Maybe it’s the point I don’t have. I am thinking “What is the difference between sciences based or not” should be the main purpose of my visit.
By the way, I am researching the mechanical friction the sole of the foot. By using friction mater, forefoot and hind foot friction resistance direction is opposite. During gait analysis at the moment of Initial Heel contact, hind foot skin must resist to the posterior GRF but after Terminal Stance forefoot skin must resist anterior GRF. Additionally forefoot skin do not resist to posterior direction during usual gait. These things may be mechanically natural, nevertheless I am not good at the evidence based thinking. All of three professors have lots of experience about scientific thinking and know how to proceed the research scientifically. I would like to decide what I should do for the rest of my life as a physical therapist. But after listening her educative talk to the graduate students, I sensed I have the motivation to make my research scientifically. So what I ask you are take part in your lab meeting and would like to have lot of awareness. And please have time to listen my techniques and criticize it. It will also be my purpose of visit here. If possible it would be honor for me that I would have lots of chances to talk to them.That’s what I would like to do. In front of my apartment, I am able to see the football ground, the sound from the ground give me lots of energy. My mission which I decide the purpose for rest of my life will be just around the corner.
